Office Information: Medical Forms

On your first visit, we ask that you complete a brief medical history and registration form. Also, please bring any pertinent information with you including records, x-rays, and insurance cards. A permanent record will be made during this time. Please inform us if you have been referred to Waco Orthopedic and Sports Medicine Clinic by another physician so we may report to them promptly.
